We are seeking a Registered Nurse (RN) Case Manager in the Tucson Area who is passionate about hospice care and committed to delivering excellence in symptom management, patient advocacy, and family support. As an RN Case Manager, you will utilize your clinical expertise to provide comprehensive, holistic care to hospice patients and their families. This role emphasizes autonomy, critical thinking, and compassionate communication.

• Perform comprehensive nursing assessments and ongoing patient evaluations
• Develop and implement individualized plans of care focused on comfort and quality of life
• Provide education and support to patients, families, and caregivers
• Manage symptoms, including pain and complex disease processes
• Collaborate with the interdisciplinary team to ensure coordinated, high-quality care
• Advocate for patient needs and uphold hospice philosophy of care

• Active Registered Nurse (RN) license in Arizona
• Minimum of 2 years of nursing experience preferred
• Hospice, home health, or case management experience strongly preferred
• Strong assessment, clinical judgment, and communication skills
• Experience with electronic health records (EHR) required
• Spanish-speaking preferred
