As the RN Memory Care Triage at Banner Alzheimer’s Institute, you will play a vital role in supporting Memory Care Neurologists by serving as the primary liaison between patients, families, and healthcare providers.
Your responsibilities will include triaging patients to assess their needs, managing prior authorizations to ensure timely access to treatments and medications, and coordinating seamlessly with neurologists and the care team to deliver comprehensive memory care services. You will utilize systems such as Cerner, Rev Cycle, and Cover My Meds to streamline workflows, process medication approvals, and maintain accurate patient records. This position requires strong clinical judgment, excellent communication skills, and a compassionate approach to working with patients and families navigating Alzheimer’s disease and related dementias, ensuring they receive the highest quality of care and support throughout their treatment journey.

POSITIONSUMMARY
This position assesses, plans, implements, evaluates and documents nursing care of patients in accordance with organizational policies and in accordance with standards of professional nursing practice utilizing the framework for professional nursing practice and development. This position is accountable for the quality of nursing services delivered by self or others who are under their direction. This position utilizes specialized knowledge, judgment, and nursing skills necessary to assess data and plan, provide and evaluate care appropriate to the physical and developmental age of assigned patients.
COREFUNCTIONS
Plans care in collaboration with members of the multidisciplinary team. Reviews and revises the plan of care to reflect changing patient needs based on evaluation of the patient’s status.
