- Assess patients’ plans of care and develop, implement, monitor, and document resource utilization and patient progress
- Evaluate medical necessity and appropriateness of care to optimize patient outcomes
- Serve as liaison for referring physicians and assist with scheduling initial tests and consultations
- Assist patients and families through diagnostic services, treatment, and care
- Coordinate disease care using an interdisciplinary holistic approach and make appropriate referrals and consultations
- Educate patients and families about diagnostic processes, care strategies, disease processes, expected side effects, and community resources
- Evaluate functional abilities and limitations, establish treatment goals, and implement plans of care
- Initiate discharge planning and recommend modifications to referring orders
- Support patients and families during difficult decision-making and coordinate end-of-life care
- Provide emotional support
- Support process improvement activities and participate in staff development
- Maintain collaborative relationships with physicians, payers, and healthcare team members
- Collect and communicate timely information to payers and other stakeholders for utilization and regulatory requirements
- Independently coordinate patient care across facilities and specialty practices
- Solve complex problems related to patient evaluation, treatment, and discharge planning
- Bachelor's degree in nursing or related field
- Current RN license in the state of practice
- BLS certification
- Typically 5 years of clinical experience
- Excellent organizational skills
- Clinical knowledge regarding specialty care services
- Knowledge of care coordination of services and legal and financial aspects of diagnostic and health services in the specialty area
- Effective communication and writing skills
- Good time management skills
- Knowledge of word processing and database software applications
- Ability to teach clinical and non-clinical personnel regarding care and diagnostic services
- Understanding of process improvement
- Current certification in specialty area preferred
- Prior Case Management experience preferred
- Additional related education and/or experience preferred
Demonstrates expertise in patient care coordination, including assessment, treatment planning, and interdisciplinary collaboration. Proficient in evaluating medical necessity, optimizing patient outcomes, and providing education to patients and families.
