RN - Home Health

This role includes 8-hour shifts, from 8:00 AM to 4:00 PM, Monday through Friday.

We are seeking a dedicated Home Care Nurse with a minimum of 24 months of home care experience. The selected candidate will provide exceptional patient care and support in home settings, adhering to the highest clinical standards. This position requires a commitment to patient-focused care and collaboration with healthcare teams.

• Provide comprehensive nursing care to patients in their homes.
• Administer medications and treatments as prescribed.
• Conduct patient assessments and develop care plans.
• Coordinate with healthcare professionals to ensure effective patient outcomes.
• Educate patients and families on health management and care techniques.

• Minimum of 24 months of home care experience, with recent experience in the last 12 months.
• Active nursing license with no hits on state license to practice.
• Life support certifications must be from AHA, ARC, or ASHI.
• Driver's license to verify residency.
• Travel experience is highly preferred.
