JOB DESCRIPTION

Case Managers coordinate in-home and community-based services for the elderly and people living with all types of disabilities.

Duties to include:
  • Working Mon-Fri 8a-5p. Position may occasionally require a slightly different schedule to accommodate client needs, including early or late shifts, evenings, and weekends.
  • Performing on-call duties at least 4-5 times per year. These duties include answering and managing emergency phone calls from 7p-7a M-F and all day Sat and Sun. These duties may also include working direct care shifts.
  • Maintaining and growing a caseload.
  • Scheduling shifts for clients and direct care staff.
  • Orienting and training direct care staff with clients.
  • Monitoring direct care staff performance.
  • Holding staff meetings as needed and attending weekly department meetings.
  • Conducting intake of new clients.
  • Meeting with clients/families in their homes minimally every quarter.
  • Attending meetings with other agencies involved in a client's care.
  • Ensuring that all paperwork in client files is updated regularly.
  • Documenting all significant conversations and/or correspondences with staff and clients.
  • Maintaining client and agency confidentiality at all times.
  • QUALIFICATIONS
  • 18 years of age or older.
  • Ability to obtain a current CPR/FA certification.
  • Ability to obtain fingerprint clearance.
  • Ability to clear a DES background check.
  • Ability to obtain a current T.B. clearance card.
  • The ability to work in a team environment.
  • The ability to handle difficult situations with clients/families.
  • The ability to work a flexible schedule.
  • The ability to learn and operate UCPSA program software.
  • The ability to work independently.
  • The ability to lift and transfer clients (up to 50 lbs).
  • Own/have regular access to and be able to operate a motor vehicle for the purpose of visiting and transporting clients, attending meetings outside of the office, etc. Proof of current registration and insurance of the vehicle must be kept on file with UCPSA.
  • At least one year of direct care experience, personally or professionally, a degree in a related field, and/or case management experience.
  • Bilingual in English and Spanish.
