* Builds and maintains relationships with community partners and stakeholders.
* Meets with clients to gather basic information including but not limited to perceived needs, home income, health insurance information, primary care information and case worker information.
* Assesses client needs and coordinates services with community resources.
* Provides information or refers individuals to community services for assistance.
* Keeps records and prepares reports.
* Performs all other duties and tasks as assigned.

** Indoor and outdoor work with some exposure to physical, ergonomic, or emotional hazardous environment.
** Position Specific Summary
** The Community Services Outreach Specialist at the City of Tucson’s Fire Department is responsible for managing a caseload of 15-20 clients. This includes initial efforts to connect each individual to community resources to fill gaps in service. Also responsible for regularly checking on each client to ensure that they are successful with the plan that is built for them.
