Position: Family Connection Consultant/ Nurturing Parenting Practitioner

General Description

A Family Connection Consultant (FCC) / Nurturing Parenting Practitioner (NPP) works with families and their children through home or virtual visitation to deliver the Family Connections and/or Nurturing Parenting Curricula. They provide individualized services to best meet the family needs in accordance with the model and curricula, meeting families at times convenient to them within the stipulated periods. The FCC/NPP maintains a caseload of 6–10 families, delivers weekly visits, and completes data entry within established time frames.

  • Must have a bachelor’s degree or higher in social work or a related field and a minimum of one year of experience in social services.
Summary of Essential Job Functions
  • Implement the Family Connections model and Nurturing Parenting curriculum with fidelity, following all program and agency policies and procedures, and DCS Standards of Practice.
  • Conduct virtual and home visits, maintaining a caseload of 6–10 families.
  • Complete individual visits weekly and deliver the FC/NP curriculum as scheduled with the Program Director/Supervisor.
  • Prepare for visits and complete documentation within 48 hours of completion.
  • Meet families at times that are convenient, including evenings and weekends.
  • Complete all post‑visit documentation within 48 hours.
  • Maintain case records and data systems in accordance with agency and program policies.
  • Initiate contact with referred families within 24 hours and actively engage them in the program.
  • Travel to home visits, program trainings, and events using own or agency vehicle, and attend mandated virtual or in‑person trainings, which may include out‑of‑state travel.
  • Connect families to resources addressing their needs.
  • Maintain positive working relationships with colleagues, administration, collaborative partners, and the community.
  • Participate in weekly individual reflective supervision (1 hour) and a minimum of one hour of group supervision weekly.
  • Attend all mandatory conferences, seminars, in‑service training, and program meetings.
  • Participate in Continuous Quality Improvement activities such as file review and provide constructive feedback on the platform.
  • Build and maintain relationships with community agencies and partners to share program information and learn about resources available to families.
  • Follow agency dress code and overall policies, including when delivering virtual services.
  • Adhere to health safety policies and procedures.
  • Maintain a safe, clean, and clutter‑free work environment, including following fire codes.
  • Maintain professionalism at all times, identify, and develop professional goals with the Program Director.
  • Perform other duties as assigned by the Program Director.
Requirements
  • Minimum of a bachelor’s degree in Social Work, Family Studies, Early Childhood Education, or related field.
  • Knowledge/experience in home visitation.
  • Knowledge/experience in parenting and child development.
  • Bilingual (English/Spanish) skills preferred.
  • Knowledge of community resources.
  • Experience working with culturally and linguistically diverse populations.
  • Strong oral and written communication skills with families, social service providers, collaborators, coworkers, and the Program Director.
  • Ability to work flexible hours, including evenings and some weekends.
  • Ability to work effectively as part of a team.
  • Compliance with all agency and program policies and procedures, including confidentiality, employee conduct, computer usage, and dress code.
  • Meeting program/department performance and productivity standards.
  • Attendance at required agency and program/department meetings.
  • Adherence to schedule agreed upon with the supervisor.
  • Follow and model CFR’s core values.
  • Participation in PQI initiatives.
  • Eligibility to obtain a Level I fingerprint card.
  • Valid Arizona driver’s license, own vehicle, and meet agency liability insurance requirements.
Ability Requirements
  • Sitting for extended periods daily; standing and walking short distances.
  • Moderate twisting, bending, stooping, and lifting up to 40 pounds.
  • Use of a variety of office equipment and machines.
  • Normal manual dexterity and eye‑hand coordination.
  • Repeated hand‑wrist movement.
