What You’ll Do

The Guia is responsible for a panel of patients and works with a multidisciplinary team to provide preventive, chronic, and acute care. The role centers on building trusting relationships, meeting social needs, and promoting healthy behaviors.

  • Provide comprehensive care coordination for an assigned caseload.
  • Collaborate with patients, families, caregivers, healthcare providers, and external partners to address complex social needs.
  • Promote communication among health‑care teams and families.
  • Intervene with patients and families regarding emotional, social, and financial consequences.
  • Conduct in‑person home visits as needed, following the Home Safety Measures Policy.
  • Access and mobilize community resources to meet social care needs.
  • Document all interventions accurately in the medical record, including clinic visits, home visits, telephonic engagement, or texting.
  • Onboard patients to the Suvida model and support medical/social visits.
  • Provide patient education on acute and chronic disease management.
  • Offer guidance and confidence‑promoting techniques to help patients self‑manage health.
  • Communicate with patients in‑person, by phone, video, or text.
  • Collaborate with the multidisciplinary team, including Guia Manager, Transitions of Care managers, and Medicaid case managers.
  • Maintain knowledge of Medicare, Medicaid, and other program benefits to assist patients with resource allocation.
  • Consult and collaborate with other Guia staff on patients requiring significant community resources.
  • Coordinate care across the continuum (schedule appointments, coordinate referrals, share information).
  • Participate in daily operations of the primary care practice (answer phone calls, maintain clinical space, track enrollment and program progress).
  • Perform other duties as assigned by the Guia Manager.
What You’ll Bring

Knowledge, Skills, and Abilities

  • Prior experience assisting patients with LIS, PAP, or other benefits applications (strongly preferred).
  • 4–5 years of healthcare or social service experience with an emphasis on care coordination or resource navigation.
  • Proficiency in EMR use, Microsoft Office/Google Suite, and spreadsheet tracking.
  • Experience coordinating activities across multiple sites or teams (communication, workflow organization, goal tracking).
  • Experience managing or supporting performance goals across multiple clinical locations.
  • Fluency in English and Spanish (bilingual required).
  • Strong organizational skills and ability to manage high volume during peak renewal seasons.
  • Compassionate communication and ability to work effectively across cultures.
Education, Experience, Licensure, or Certification Requirements
  • CHW certification (preferred).
  • Bachelor’s degree (preferred).
