Purpose:
This position provides in-home case management services to individuals living in Pima County who are eligible for assistance under the U.S. Department of Housing and Urban Development (HUD) Older Adults Home Modification Program (OAHMP), administered by PCOA as the Aging Well at Home (AWAH) program. The program provides low-cost, low-barrier, high-impact home modifications and limited home repairs that enable eligible low-income older adult homeowners (age 62 and older) to age in place safely in their own homes.

Healthy Aging Manager Schedule, Post of Duty, & Travel Requirements:
37.5 hours weekly, primarily Monday-Friday, 8:30 a.m. – 5:00 p.m.This position follows a hybrid work arrangement, combining in-office, remote, and field work, as approved by the supervisor.
Frequent travel is required between clients’ homes and PCOA location.

From $23.07 (Dependent upon relevant education, experience, and preferred qualifications).Major
Duties & Responsibilities:
Screens applicants and determines eligibility per HUD Older Adults Home Modification Program (OAHMP) requirements, verifying age (62+), home ownership of a primary residence in the service area, and household income against current HUD income limits.
Travels throughout Pima County to conduct in-home assessments, including home safety, fall-risk, and functional assessments that inform recommended home modifications.
Partners with clients to develop person-centered service and home modification plans; monitors satisfaction and adjusts services as needed.
Coordinates with Occupational Therapists (OT/OTA) and contracted providers from assessment through completion and verification of work.
Conducts follow-up visits to verify completed modifications, evaluate satisfaction, and collect outcome measures required for HUD reporting.
Communicates promptly and professionally with clients, families, providers, and community partners.
Adheres to contract and grant guidelines, including mandatory contacts, data entry, per-household funding limits, allowable repair scopes, and HUD environmental review requirements.
Maintains accurate, timely client files and program records per HUD OAHMP grant requirements.
Refers and assists clients in accessing resources within PCOA and the community, including transitions to other levels of care.
Works independently with minimal supervision, collaborates effectively, attends required meetings and trainings, and ensures all work complies with applicable regulations and HUD grant terms.
Ensures that work adheres to local, state, and federal governmental regulations, and other applicable rules and requirements, including HUD OAHMP grant terms, notices, and guidance.
Uses virtual collaboration platforms to perform work as necessary (e.g. Zoom or Teams).
Job Qualifications:
An equivalent combination of education and experience related to the position may be considered.
A bachelor's degree in social work, Human Services, or a closely related field.
At least two (2) years of experience in case management and/or social services; three (3) years is preferred.
Bilingual in English and Spanish is preferred.
Additional Qualifications:
Certified Aging in Place Specialist (CAPS) certification required (CAPS training provided upon hire).Must have reliable transportation.
Must successfully obtain and maintain AZ Level 1 Fingerprint Clearance and Central Registry background clearance.
Knowledge, Skills, & Abilities:
Knowledge of case management/care coordination practices, rules, regulations, and requirements.
Knowledge of the aging process, issues, and health problems associated with aging and disabilities.
Skill in using Microsoft Office suite and virtual collaboration platforms.
Ability to demonstrate a commitment to the vision, mission, and values of Pima Council on Aging and its family of non-profit companies.
Ability to effectively adapt and maintain composure in high-stress situations.
Ability to effectively communicate with, receive information from, and convey information to others.
Ability to establish clear professional boundaries with clients.
