Summary

Outreach Engagement Specialist within the Trauma Recovery Center is responsible for engaging individuals who have been identified by hospitals, emergency personnel, families, faith communities, and other social service agencies as appropriate referrals for TRC services. The TRC Outreach Engagement Specialist will have a passion for working with survivors of violent crime/trauma such as sexual assault, domestic violence, physical assault, shootings, gang violence, vehicular assault, human trafficking and family members of homicide victims.

The population served focuses on underserved and at-risk populations such as persons who are unhoused, from refugee populations, and are of diverse ethnicity or origin; as well as who have chronic mental illness, substance use disorders, disabilities, and severe trauma-related symptoms or complex psychological issues.

  • Perform assertive community outreach with/at hospitals (including, but not limited to Emergency Departments), other victims’ service organizations, homeless camps, shelters, detox centers, faith communities, police departments and other locations/partners to engage individuals who are appropriate for TRC services.
  • Build and maintain professional relationships with local community groups, service organizations, hospitals, police, jails, courts, etc.
  • As part of the TRC treatment team, provide assertive community outreach and clinical case management consistent with treatment plan.
  • Helps member to complete eligibility applications for various financial entitlements.
  • Provides outreach and support to the member’s home, in the office, and in community-based settings, as needed. Accompanies member to appointments and interviews with appropriate agencies for victims of crime wishing to file police reports, restraining orders, etc. (such as Police/Sheriff’s Department, Pima County Attorney’s Office, etc.).
  • Provide follow up on all referrals to assess outcomes and additional services as required.
  • Conducts initial screening of individuals for eligibility for Trauma Recovery Center services and makes appropriate referrals.
  • Coordinates intake and enrollment with the appropriate CODAC staff by supporting individual until enrollment is completed.
  • Assesses and addresses barriers to treatment engagement.
  • Tracks and maintain written records of community contacts and outreach for potential engagement.
  • Consults with and cooperates with community systems to facilitate linkage, referral, crisis management, advocacy, and follow up with the focus on attaining treatment goals.
  • Provides support and/or assistance to other departments to ensure service engagement for the Trauma Recovery Center.
  • Assists in wellness and reengagement outreach via telephone, home visits and other viable means.
  • Transports prospective members to site for intake and to local service organizations, for medical, legal, law enforcement and other needed services, as appropriate.
  • Provides crisis intervention and de-escalation of members, as needed.
  • Attends staff meetings and scheduled trainings.
  • Perform other related duties in accordance with agency growth and changes.
