Job Description
Adventist Health Tulare is a 108-bed acute care hospital located in Central California and has been serving the Central Valley since 2018. We provide primary and specialty care services to the community. Tulare is situated in a geographically diverse region with year‑round outdoor recreation and is within an hour's travel to Yosemite, Sequoia, and Kings Canyon National Parks.
Job SummaryMaintains responsibility for clinical and educational practices of qualifying patients and protocols. Supervises the care program, performs daily rounds on inpatients, monitors patient progress through the continuum of care, and tracks key indicators to ensure Joint Commission compliance for evidence‑based patient safety practices. Provides care coordination, care plan development, and skilled nursing assessment. Works with surgeons to ensure patients/families understand the procedure, condition, and post‑surgery care plan.

- Anticipates and assists with symptom management, including pain control. Identifies opportunities to improve patient/family experience and resource efficiency. Participates in family support, teaching, discharge planning and continuity of care across sites in collaboration with the primary care team. Participates in formal/informal education for all disciplines. Leads a multidisciplinary team meeting monthly and serves as a resource for frontline staff education.
- Maintains accountability for key performance measures; works with team to identify/track measures such as patient satisfaction, physician satisfaction, length‑of‑stay and discharge disposition. Distributes reports to stakeholders and participates in process improvement initiatives.
- Functions as a member of an integrated professional team in accurate assessment of patient needs through rounding with physicians/nurse caregivers to develop, implement and evaluate the nursing care plan. Develops and revises patient education materials and promotes evidence‑based practice through research review.
- Ensures all materials are standardized and consistent with order sets and clinical pathways. Updates with physicians/staff annually or as needed to maintain high quality of care standards and promotes evidence‑based practice.
- Provides support to the team for continual learning, participates in staff education on an individual or group basis, serves as a resource to all staff regarding care provision, establishes appropriate care standards, and sets realistic goals for optimal patient outcomes. Keeps current with changes in specialty care and service and incorporates into performance systems.
