Job Summary
The Intake Coordinator will be the first encounter and essentially the first impression to the stakeholders, participants, and families. The Intake Coordinator is accountable to and under the direction of the PACE Executive Director of Operations and manages the overall intake efforts including facilitating tours in the center as well as tracking and collecting data on enrollments and disenrollments.
Duties/Responsibilities- Answer incoming calls/faxes from referrals and potential clients interested in the PACE program.
- Communicate and exchange information with internal and external sources via writing, in person, by telephone, or by email.
- Meet goals related to growth and census for PACE.
- Establish and maintain cooperative relationships with representatives of the community, consumers, and employees.
- Develop weekly goals with the leadership team to prioritize, organize, and accomplish initiatives regarding development and cross-maximization of referral sources.
- Coordinate each step of the intake and enrollment processes.
- Clearly communicate program and protocols to qualified leads and families.
- Facilitate tours and assist with program education as needed.
- Perform new participant orientation, including creation and issuance of membership cards.
- Track enrollment progress jointly with intake team members and report progress to the Executive Director of Operations.
- Maintain documentation of daily contacts and assist in analyzing monthly referral tracking reports for effectiveness and emerging trends.
- Attend marketing, staff, and IDT meetings as required.
- Enter intake documentation into the EMR system.
- Activate enrollment status and disenrollment status as applicable.
- Provide backup to administrative professionals.
- Track and report mileage and expenses using HR software in a timely manner.
- Document all touchpoints in EMR.
- Complete any additional duties and responsibilities as assigned.

Valid Driver’s License
Education:
Bachelor’s degree in a related field or 3 years of relevant experience.
