This position in integrated care program (mental health, substance abuse, medical) is a key member of team-based care for primarily adults and some children with co-occurring needs. RN Care Manager conducts assessments, coordinates care, meets individually and in group sessions to educate clients on health and allover wellness needs.
Responsibilities- Conduct comprehensive assessment of psychiatric and medical history (e.g. onset, course and effect of illness, past treatment and responses, and risk behaviors), mental status, and diagnosis; physical health and dental health; use of drugs or alcohol; education and employment; social development and functioning; activities of daily living (e.g. self-care, living situation, nutrition, money management); and family structure and relationships.
- Provide care management for consumers including coordinating and monitoring the activities of the CCBHC Treatment Team; contribute to developing, writing, implementing, evaluating, and revising overall treatment goals and plans, providing individual supportive sessions and symptom management, educating and supporting consumers’ families, and advocating for consumers’ rights and preferences.
- Collect data at designated intervals in session with clients to document progress, needs, barriers and demographics by established deadlines. Prioritizes data collection.
- Document consumer progress to maintain a permanent record according to CCBHC protocols.
- Participate in providing rehabilitation, comprehensive care management, care coordination, health promotion and education, comprehensive transitional care, and support services for clients and their families.
RN licensed to practice medicine in the State of Oklahoma. Bilingual a plus.
Skills and KnowledgePreferred- 1 to 3 years of experience in a behavioral health setting. Use of electronic health record and various technology required, training provided.
Must possess Oklahoma Driver License and use personal automobile for local travel to locations other than primary office. Office-based and field-based services are provided. Currently working remotely from home providing telephone and telehealth (video) services due to COVID
19 protocols, will return to in-office work in the future.

You will be working in an innovative and dynamic environment using a new transformative model of care Certified Community Behavioral Health Clinic (CCBHC). This model is characterized by innovative, team-based approach and whole person care for adults and children with a range of complex mental health and substance use challenges.
Utilizing multi-disciplinary teams, staff work collaboratively to provide a coordinated effort to enhance client recovery. Robust CCBHC services often result in better client outcomes and quality of care due to:
- Increased access to care and crisis services
- Expanded traditional community mental health and substance use services.
- Added Care Coordination and physical health screening for mental health clients
- Greater access to Social Services for clients’ economic and social needs
- Increased specialized services for veterans, those most in need, and those impacted by the opioid crisis.

EducationAssociates or better in Nursing or related field.
