About This Position

The Perinatal Nurse Navigator supports pregnant and postpartum patients through a hybrid model of virtual and in-person care coordination across OSU Medicine Virtual Care’s rural hospital and clinic partners. This role focuses on improving maternal and perinatal outcomes through care coordination, postpartum engagement, high-risk referral management, and support of transitions of care from pregnancy through the postpartum period.

The navigator works closely with obstetrics, maternal-fetal medicine, behavioral health, addiction medicine, primary care, and rural hospital teams. The role includes patient navigation, education, remote monitoring support, referral management, and protocol-based clinical follow-up visits under physician/APRN supervision.

May have hybrid work options included some travel.
