Tulsa, OK - Primary Care NP or PA position

JOB DESCRIPTION

Elaya Health is seeking experienced Physician Assistants or Nurse Practitioners (PAs or NPs) with Internal medicine or primary care experience.

Primary care isthe first contact and principal point of continuing care for residents in the skilled nursing facility (SNF) or Assisted living facility (ALF). You will be responsible for acute and chronic physical,mental,and social health issues, including multiple chronic diseases such as hypertension,angina,diabetes,asthma,COPD,depression,and anxiety,back pain,arthritis,thyroid dysfunction.

As an NP/PA clinician, you will collaborate with the nursing team to provide exceptional care to the residents in the facility. This position involves treating patients exclusively at a designated skilled nursing facility or assisted living facility.

Responsibilities:

• PA or NP will provide primary care services as an outside consultant in SNF or ALF
• Work alongside onsite nursing staff to improve patient function and quality of life

Qualifications:

• Graduate of an accredited Physician Assistant Program or Nursing School
• SNF or post-acute experience (preferred)
• 6 Months Work Experience
• Registered as a Physician Assistant or Nurse Practitioner in Oklahoma State
