Community Relations Manager

Oak Street Health seeks a Community Relations Manager to expand access to high-quality primary care for adults on Medicare in underserved neighborhoods. In this role, you will design and lead outreach strategies, build partnerships with community organizations and senior centers, and host educational events and health fairs. Working closely with clinical and operations teams, you will connect community members to preventative care, track outreach impact, and continuously refine engagement efforts.

Responsibilities

• Develop and execute community outreach strategies to increase awareness of Oak Street Health services.
• Build and maintain relationships with community organizations, senior centers, and local leaders.
• Plan and host educational events, health fairs, and workshops for Medicare-eligible adults.
• Coordinate patient engagement initiatives that support access to preventative care.
• Track outreach metrics, prepare reports, and adapt strategies based on results.
• Collaborate with clinical and operations teams to align outreach with care goals.
• Represent Oak Street Health at community meetings and public events.
• Support marketing campaigns and referral programs within the local market.
