As a Chaplain, you provide spiritual and emotional support to patients and families throughout their hospice journey.

Responsibilities include:

• Conducting spiritual assessments and offering individualized support
• Assisting with bereavement services as needed
• Participating in the interdisciplinary Plan of Care to ensure high-quality spiritual care
• Maintaining accurate and timely documentation in patient charts
• Building relationships with community religious and spiritual leaders
• Coordinating access to community spiritual resources for patients and families
• Supporting the development of hospice-community partnerships

• Ordained minister or chaplain in good standing within your denomination
