NEW Registered Nurse (RN) - Home Health | Direct Hire
Seeking a compassionate and experienced Registered Nurse to provide skilled home health nursing care. This role focuses on patient assessment, care planning, medication management, coordination with physicians and care teams, and patient/family education in the home setting.
Key Responsibilities:
- Assess patient eligibility and needs for home health services
- Develop, implement, and revise individualized plans of care
- Provide skilled nursing care considering physical, emotional, cultural, and social factors
- Coordinate care with physicians, therapists, aides, and interdisciplinary teams
- Perform medication reconciliation and identify potential drug interactions or compliance issues
- Educate patients and caregivers on disease management, safety, and emergency care
- Complete accurate and timely documentation in the EMR
- Conduct Home Health Aide supervisory visits every 14 days
- Support quality assurance, PDGM case conferencing, and performance improvement initiatives
- Ensure infection control and agency policies are followed
- Active RN license (multi-state or state-specific)
- Home Health nursing experience required
- Strong clinical judgment, communication, and organizational skills
- Basic computer proficiency and EMR documentation experience
- Valid driver's license, reliable vehicle, and acceptable driving record
- Direct Hire | Full-Time
- Schedule:
Monday-Friday, Day Shift (flexible to patient needs) - Pay Range: $33 - $46 per hour
- Mileage reimbursement: $0.70/mile
- Sign-On Bonus: $10,000
