Overview

The Hospice Nurse Practitioner - Exempt is responsible for modeling the Compassus values of Compassion, Integrity, Excellence, Teamwork, and Innovation and for promoting the Compassus philosophy, using the 6 Pillars of Success as the foundation. S/he upholds the Code of Ethical Conduct and promotes positive working relationships within the company, with all departments and external stakeholders. The Nurse Practitioner provides palliative or hospice care as assigned, including assessments, education, and symptom management to meet patient and family needs, in collaboration with the PCP, specialists, and the Medical Director.

S/he may be required to provide on-call support and after-hours emergency visits and/or admissions.

Position Specific Responsibilities
  • Performs routine and emergency assessments, including recertification visits and admissions/transfers as applicable; records observations and communicates findings to Medical Director, Attending Physician, DCS, and IDT; provides physical and emotional care during crises.
  • Act as attending practitioner for patients who select them at admission; collaborate with patient, family, IDT and physician on symptom management and end-of-life care.
  • Rotates into on-call schedule when required with physician backup for admission certification.
  • Prevent unwarranted hospitalizations by meeting patient and family needs at home; for outpatient palliative care: perform routine and emergency assessments, bedside evaluations, diagnose/treat/manage conditions, order tests and treatments, prescribe medications (including controlled substances where licensed), coordinate care, and maintain records for reimbursement and compliance.
  • Collaborates/communicates with Medical Director, Attending Physician, DCS, and IDT; document and submit all required paperwork within 24 hours of visit.
  • Educates patient and caregiver regarding care, disease process, dying process, symptom control, wound care, and treatment options.
  • Engages in care coordination for palliative and hospice referrals; participates in development of clinical practice guidelines and standards; maintains confidentiality.
  • If providing Hospice Face-to-Face Visits: perform routine and emergency assessments and related documentation; coordinate with IDT and physicians for ongoing care.
  • If providing Palliative Care: perform routine and urgent assessments, triage consultations by patient acuity, educate patients/families, order tests and treatments, collaborate with physicians, and manage records and billing per CMS guidelines.
  • Define goals for professional growth and participate in professional activities; engage in self-care and adhere to confidentiality.
  • Collaborates/Communicates with Medical Director, Attending Physician, hospital staff, and IDT; participate in monthly IDT meetings where applicable.
Education And/Or Experience
  • Master's degree in Nursing required.
  • Hospice Care:
    Minimum of three (3) years nursing practice or equivalent required; hospice, oncology, home health, or long-term care preferred.
  • Palliative Care:
    Minimum of three (3) years nursing experience required; clinical nursing care in oncology, hospice, geriatrics, or ICU preferred.

  • Mathematical

    Skills:

    Able to perform basic arithmetic and unit conversions.
  • Language

    Skills:

    Strong reading, writing, and communication abilities; capable of presenting information and responding to questions.
  • Other

    Skills:

    Fluent in English; strong organizational and time management skills; flexible schedule including nights/weekends; ability to travel; maintain patient information confidentiality.
Certifications, Licenses, and Registrations
  • Active and unencumbered CNS or Nurse Practitioner license in state of employment.
  • Certified as a Nurse Practitioner in Adult, Family, Acute, or Geriatric required.
  • Active NPI number.
  • DEA certification if state allows prescription of controlled substances by APPs (maintain active).
  • Board Certification in Hospice and Palliative Care preferred, but not required.
Other
  • State-specific requirements:
    All states except Iowa require a current collaborative physician agreement.
Physical Demands and Work Environment

The role requires standing, walking, manual dexterity, lifting/moving up to 50 pounds, and varying visual demands. Exposure to bodily fluids, infectious diseases, and clinical conditions is possible. Regular use of standard clinical equipment is expected. Adaptations can be made for disabilities as needed.
