A Path of Care Home Health offers skilled care and support to our patients so they can live safely and independently in their own homes. We work directly with doctors to develop individualized care plans that our nurses, therapists, and aides follow to ensure our patients receive the best care possible to heal. In every interaction with our customers, we strive to demonstrate CARE, which stands for our core values of credibility, accountability, responsiveness, and empathy.
We recognize that lives may be dependent on our willingness and ability to respond quickly, which is why we strive to understand the special needs of our patients and bring sunshine to their lives. We work hard to create a supportive and fulfilling environment that helps our employees grow professionally and create long‑lasting careers they can take pride in.
As an LPN in home health care, you play an essential role in visiting patients in their homes to ensure they receive high‑quality care that meets their specific needs. When meeting with patients, you provide hands‑on nursing skills, including injections and wound care while ensuring equipment and materials are prepared correctly. You also teach your patients appropriate self‑care techniques, so they have some control over their own health.
Through communication with RNs and case managers, you provide updates on each patient and clarify instructions when unclear.
In addition, you aid RNs or physicians in performing specialized procedures and duties as well as carrying out any oral or written instructions that come your way. To ensure the safety of your patients, you practice infection control and universal precautions. When needed, you report any forms of abuse and take appropriate action according to state and federal guidelines.
