We are seeking a Hospice RN Case Manager who is passionate about making a meaningful difference in the lives of patients and their loved ones.

Position Summary

The Hospice RN Case Manager is responsible for coordinating and managing the care of hospice patients, ensuring quality, continuity, and compassionate support throughout their journey. This role works collaboratively with the interdisciplinary team to deliver exceptional care aligned with the patient’s goals and needs.

Key Responsibilities
  • Manage a caseload of hospice patients across home and facility settings
  • Perform comprehensive assessments and develop individualized care plans
  • Coordinate care with physicians, social workers, chaplains, aides, and volunteers
  • Provide skilled nursing care, symptom management, and patient/family education
  • Participate in Interdisciplinary Team (IDT) meetings
  • Ensure accurate and timely documentation in compliance with CMS/CHAP standards
  • Provide on‑call support as assigned
  • Build meaningful relationships with patients, families, and community partners
Qualifications
  • Current Registered Nurse (RN) license in the state of Oklahoma
  • Minimum 1 year of nursing experience (hospice or home health preferred)
  • Strong clinical assessment and critical thinking skills
  • Excellent communication and compassionate bedside manner
  • Ability to work independently while collaborating with a team
  • Valid driver’s license and reliable transportation
