• Provide direct care for patients with chronic and acute conditions
• Collaborate with physicians to guide treatment plans
• Partner with facility leadership and interdisciplinary teams
• Help support Med Cap Health's rollout into a new community

• 1+ years of experience in post-acute/SNF/LTC
• Board-certified geriatric, family, or adult Nurse Practitioner (ANCC, ACHPN, or AANP)
• Post-acute, long-term, and/or skilled nursing facility experience

Pay: This contract role offers a highly competitive annual range of $150,000 to $200,000
