Registered Nurse Home Health at Elara Caring

• Ensure that all activities performed align with Elara Caring’s mission and leadership expectations.
• Maintain patient and staff privacy and confidentiality pursuant to HIPAA Privacy Final Rule.
• Assess assigned caseload of home health care patients and families to identify physical, psychosocial, and environmental needs.
• Assume primary responsibility for caseload, including assessing, planning, coordinating, implementing, and evaluating patient plans of care.
• Obtain data on physical, psychological, social, and spiritual factors that influence health status and incorporate into plan of care.
• Make the initial evaluation and re-evaluate nursing needs during each visit and schedule follow‑up visits as needed.
• Communicate significant findings, problems, and changes in health condition or environment to the Clinical Supervisor, physician, facility, and other personnel.
• Revise the plan of care in response to identified issues and notify team members.

• Current, unrestricted state RN license.
• At least 1 year of clinical nursing experience.
• Ability to travel within the branch/office coverage area and willing to travel up to 50% of the time.
• Ability to frequently lift, push, pull, and support up to 50 pounds.
• Must have a dependable vehicle, valid driver’s license, and current auto insurance.
• Associates Degree or Bachelor’s Degree in Nursing preferred.
• Previous home health or hospice experience preferred.
• Previous experience with Home Care Home Base preferred.
• You will report to the Clinical Manager or Clinical Supervisor.
