American Traveler is hiring an RN Case Manager to serve as an Interim Renal Pre-Transplant Coordinator, requiring 2+ years of recent transplant coordinator experience and an active RN license (state-specific or Compact/eNLC).

• Renal Transplant unit within a hospital setting
• Coordinates clinical evaluation, diagnostic workup, and UNOS waitlist management for kidney transplant candidates
• Schedule is Mon-Thu 8:00 AM-4:30 PM and Fri 8:00 AM-12:00 PM, totaling 36-40 hours/week
• No on-call, holiday, or weekend requirements
• Uses Epic (Phoenix module preferred) or Cerner Organ Transplant software for documentation

• Active RN license (State-specific or Compact/eNLC)
• BLS certification required (must be AHA)
• CCTC (Certified Clinical Transplant Coordinator) certification preferred
• 2+ years of recent clinical transplant coordinator experience in a CMS/UNOS-approved kidney transplant program
• Hands-on EMR experience with Epic (Phoenix module preferred) or Cerner Organ Transplant software

• Coordinate diagnostic testing, labs, and multi-specialty consults for transplant candidates
• Compile and present candidate cases at weekly Multidisciplinary Selection Committee (MSC) meetings
• Maintain UNOS/OPTN candidate status updates, annual re-evaluations, and documentation to ensure CMS compliance
• Educate candidates and families on the evaluation process and waitlist requirements
• Serve as the primary clinical liaison to referring dialysis centers and nephrologists
• Accepts local candidates, though a radius rule may apply that could affect pay rate
