Hospice Marketer / Community Liaison

Tru Heart Hospice is seeking an energetic, compassionate, and relationship-driven Hospice Marketer / Community Liaison to join our team.

The ideal candidate is passionate about hospice care, comfortable building professional relationships, and motivated to grow hospice awareness and referrals throughout the community. This position serves as a key connection between Tru Heart Hospice and hospitals, physicians, skilled nursing facilities, assisted living communities, home health agencies, senior living communities, and other healthcare partners.

Position Summary

The Hospice Marketer / Community Liaison is responsible for developing and maintaining referral relationships, educating healthcare professionals and the community regarding hospice services, identifying potential referral opportunities, and supporting appropriate and timely access to hospice care.

This individual will work closely with hospice leadership, clinical staff, and the admissions team to promote Tru Heart Hospice while maintaining the highest standards of professionalism, integrity, and compliance.

Key Responsibilities
  • Develop and maintain relationships with hospitals, physician offices, skilled nursing facilities, assisted living communities, senior living communities, home health agencies, and other referral partners.
  • Conduct routine sales and relationship-building calls within the assigned territory.
  • Educate referral sources regarding hospice philosophy, services, eligibility guidelines, and the referral/admission process.
  • Identify new referral opportunities and develop strategies to increase appropriate hospice referrals.
  • Maintain regular communication and follow-up with existing referral partners.
  • Coordinate closely with the clinical and admissions teams to help ensure timely response to referrals.
  • Represent Tru Heart Hospice at community events, healthcare networking events, facility meetings, educational programs, and professional organizations.
  • Assist with community outreach, presentations, health fairs, and hospice education.
  • Maintain accurate documentation of marketing activities, referral contacts, and business-development efforts.
  • Monitor referral trends and provide regular updates regarding territory activity, opportunities, and barriers.
  • Develop territory plans and work toward established referral and census-development goals.
  • Maintain knowledge of hospice services, Medicare hospice eligibility, and applicable marketing and compliance requirements.
  • Protect patient confidentiality and comply with HIPAA requirements.
  • Conduct all marketing activities ethically and in accordance with applicable federal and state regulations and Tru Heart Hospice policies.
Qualifications
  • Previous hospice, home health, healthcare marketing, business development, or healthcare liaison experience preferred.
  • Experience developing relationships with physicians, hospitals, skilled nursing facilities, assisted living communities, and other healthcare providers strongly preferred.
  • Knowledge of hospice services and the healthcare referral process is preferred.
  • Excellent communication, presentation, networking, and interpersonal skills.
  • Strong organizational and follow-up skills.
  • Self-motivated and comfortable working independently in the community.
  • Ability to establish professional relationships and represent Tru Heart Hospice with compassion and integrity.
  • Valid driver's license, reliable transportation, and ability to travel throughout the assigned territory.
  • Ability to attend occasional community or networking events outside normal business hours.
