Southwest Collaborative Support Services (SWCSS) is a collaborative partnership supporting children/youth and families. Partners worked together under the previous RCSD framework since 2013 and launched SWCSS in August of 2020 to continue to support the needs of students across the region. Reporting to the SWCSS Regional Coordinator, the Rural Student Support Navigator will work with three rural school divisions across Southwest Alberta to promote student wellbeing. The purpose of this role is to support school staff, empower families, and build capacity throughout the region. This program offers goal-oriented case management to support parents and enhance overall family wellbeing. This includes providing support in connecting to local community resources, applications and paperwork, and parental advocacy.

The Student Support Navigator typically works with families with Tier 3 complexity.

ABOUT THE OPPORTUNITY
  • Work schedule will follow the school calendar with Christmas, Easter and summer break observed and is based on a 35-hours work week
  • Regional team members will be provided with laptop, cell phone and a small supply budget
  • Service will be provided by a combination of in-person and virtual delivery
  • Travel costs will be reimbursed based on SWCSS approved rates
  • Specific service priorities will be determined by school division partners funding the service.
  • Service priorities are likely to be focused at the top tiers of intervention.
KEY DUTIES & RESPONSIBILITIES Family Supports
  • Build trusting and non-judgmental relationships with parents to support effective service delivery.
  • Conduct intake assessments to determine parent-led goals and individualized care plans.
  • Prioritize and manage a dynamic caseload.
  • Facilitate the coordination of external resources and guide parents through system navigation.
Complex Case
  • Chair bi-weekly Complex Case team meetings.
  • Coordinate administrative duties, including agenda preparation and minute dissemination.
  • Screen referrals for eligibility and coordinate next steps for access to the team.
  • Compile and share team recommendations with referrers.
Collaboration
  • Provide resource-based consultation to school teams, including regular attendance to division meetings.
  • Liaise with school administration, community organizations, and cultural partners.
  • Partner with multi-ministry agencies to streamline access to services for students and families.
Other Activities
  • Oversee and process new referrals.
  • Develop and present mental health materials to school-based teams.
  • Manage digital presence by maintaining current information on the organization’s website.
  • Maintain accurate and timely financial, statistical and narrative reports in accordance with organizational standards.
QUALIFICATIONS & WORK EXPERIENCE
  • A related post-secondary degree in the Social Science fields such as social work, psychology, public health;
  • At least 3 years of experience in working within the mental health field, particularly in systems navigation;
  • Proof of licensure or registration with an approved Alberta regulatory college, prior to commenting duties.
