Job Title
Fatherhood Case Manager
Job Description Within the Healthy Start guidelines and in coordination with other Healthy Start Case Managers, provides education and case management services to the fathers or partners of Healthy Start mothers and babies that strengthen the father's capabilities and that promote the Healthy Start family. Participates actively in fatherhood efforts in the Tulsa community, to include the Tulsa Fatherhood Coalition and the Healthy Start Community Action Network (CAN).

The following functions represent the majority of the duties performed by the position, but are not meant to be all-inclusive or prevent other duties from being assigned when necessary:
ESSENTIAL JOB FUNCTIONS:
Provides fatherhood case management services for 25 fathers/partners annually.Works with the mother's Healthy Start Case Manager, maintaining regular and ongoing (at least monthly) communication to ensure effective and coordinated services to the client family.Conducts a fatherhood assessment and develops a service plan, which identifies needs and goals related to employment, education, parenting, and personal health.Completes client documentation and ensures accurate database entry according to Healthy Start guidelines. Ensures HIPAA confidentiality, voluntary consent to participate, and as appropriate, release-of-information documentation.Meets HRSA Healthy Start performance standards related to number of clients served, as well as the frequency and method of contact.Attends monthly Healthy Start case staffing and case manager meetings.Secures information or materials needed to present educational materials to fathers of Healthy Start. Operates audio/visual equipment in presentation of educational programs.Works in the community to partner with existing programs, transportation connections, Workforce Tulsa, Job Corps, etc. that may hire a Tulsa Healthy Start father participant. Learns application process and job expectations that would strengthen a participant's application. Supports participant in early weeks of employment regarding specific job expectations.Works with GED and area colleges to learn application process and requirements in order to support Tulsa Healthy Start father participants. Supports participant in early weeks of classes regarding education expectations.Promotes Healthy Start's educational programs and represents the Tulsa Health Department to individuals and community groups, health fairs, local area media, school personnel, religious organizations, childcare programs, social services agencies, and healthcare providers.May assist facilitation the Tulsa Fatherhood Coalition and serve as a resource person for neighborhood networks.Consults with Healthy Start management and THD marketing department regarding appropriate educational and promotional fatherhood materials.Attends required training in support of duties and responsibilities of this position.Other duties assigned including those required to fulfill activities in support of public health emergency operations.Typical Qualifications
Education:
Baccalaureate degree in health education/health promotion education or a related field required; master's degree in health or a related field preferred.
Experience:
Three years of experience in health education, or case management, or working with low-income families/fathers. Knowledge and
Skills:
- Excellent interpersonal and good communication skills are required. Ability to obtain cooperation from clients who come from a wide range of economic, social, and cultural backgrounds.
- Skills in interviewing and engaging clients; and in working with agency staff and community resources.
- Comfortable in gathering information about and providing education related to reproductive health.
- Ability to work closely and cooperatively with programmatic co-workers toward a Healthy Start family's goals.
- Public speaking, including radio/television interviews.
- Skill in building community relationships.
- Knowledge of community resources and helping relationships that would assist fathers and/or families.
Licenses:- Valid Oklahoma driver's license.
