* * Responsibilities:
*** Travel between multiple patient locations throughout your shift (no single 1:1 assignments)
* Provide personal care and support services to patients
* Assist with daily living activities, including bathing, dressing, mobility support
* Offer comfort and compassion to hospice patients and their families during end-of-life care
* Monitor, report, and document patient conditions
* Collaborate with the healthcare team
* Maintain accurate patient visit records
** Requirements:
*** Current CNA certification
* Must have at least 1 year of experience
* Valid driver’s license and insured vehicle
* Previous home health and hospice experience preferred
* Experience working with geriatric patients
* Compassionate, patient-focused attitude
