Purpose StatementThe job of School and Community Resources Liaison is done for the purpose of assisting schools and ESD staff in coordinating referrals and supports for youth and their families, including behavioral health care, physical health care, education, juvenile justice, and other services available in the community as a part of the local System of Care; advocating for students and their families, coordinating services, and providing ongoing feedback to the System of Care and emphasizing the importance of assisting youth and families dealing with complex needs;
and identifying, coordinating, and providing resources for districts and staff serving students in our region.
Acts as a Liaison
- Acts as a liaison between school districts, families, and community service providers for the purpose of connecting youth with appropriate supports and resources.
Attends Meetings and Professional Development
- Attends meetings, workshops, and seminars as assigned (e.g. Drug Action Team, Thurston/Mason County partnership meetings, local coalitions, etc.) for the purpose of conveying and gathering information required to perform functions.
Builds Community Partnerships
- Builds and maintains partnerships between schools and community agencies for the purpose of reducing barriers to student engagement in education.
Compiles Documentation
- Compiles documentation for the purpose of processing data for the determination of eligibility for programs and services.
Coordinates Referrals
- Coordinates referrals and monitors follow-up services for the purpose of ensuring continuity and effectiveness.
Coordinates with Service Agencies
- Coordinates with outside service agencies and school sites for the purpose of helping parents of eligible students access services identified in the student service plan.
Distributes Materials
- Distributes materials to schools and families for the purpose of providing information on available programs and services.
Maintains Records
- Maintains records and files for the purpose of documenting activities, complying with federal, state, and school system requirements and evaluation processes.
Participates in Work groups
- Participates in partner work groups for the purpose of developing and enhancing local resources to respond to unmet needs.
Prepares Materials
- Prepares a variety of materials (e.g. minutes, correspondence, narrative reports, work plans, etc.) for the purpose of providing information and written documentation.
Presents Information
- Presents a variety of information to a variety of groups and individuals (e.g. Providers Consortium, PTA, Affinity groups, parents, etc.) for the purpose of increasing collaboration between service providers.
Provides Guidance
- Provides guidance and information to personnel and partner agencies regarding available resources and services for youth and families for the purpose of increasing awareness of and access to resources.
Provides Support to Stakeholders
- Provides support to a wide variety of stakeholders (e.g. community resource coordinator, substance abuse agencies, school nurse corp, school counselors, etc.) for the purpose of coordinating and monitoring the delivery of available services.
Responds to Inquiries
- Responds to inquiries from a variety of stakeholders (e.g. parents, staff, clinicians, the public, etc.) for the purpose of providing information on community resources.
