RoleThe Host Homes Case Manager provides a high level of wraparound and social service support to Youth Participants and Host Families in the Host Homes Program. This role works collaboratively to prepare and support families in hosting youth experiencing housing instability, in addition to providing case management to youth participants with goals related to stable housing, education, employment, financial literacy, health, mental health, interpersonal relationships, and general self-sufficiency.

Assignment Duration:
The assignment is expected to last approximately five months, with an anticipated duration of fewer than six months from the actual start date.
Work Schedule:
The typical work schedule is Monday through Friday, with specific hours based on program needs. Occasional evening or weekend work may be required, as well as infrequent out-of-town travel.
Approximately one month of training and overlap is anticipated before the Case Manager assumes independent responsibility for the existing caseload.
CompensationExpected Hiring Range:
Candidates hired for this position can expect a starting wage of $27.30 – $30.00 per hour. The final starting rate will be based on relevant qualifications and experience.
Full Position Wage Range:
The established wage range for this position is $27.30 – $33.30 per hour. This broader range reflects potential wage progression over time and is not the expected starting range for new hires.
Eligible for Vacation, Paid Holidays, and Paid Sick Leave in accordance with organizational policy and applicable law.
This temporary position is not eligible for employer-sponsored medical coverage based on its anticipated duration, subject to applicable plan documents and legal requirements. Retirement plan eligibility is determined in accordance with the terms of the SIMPLE IRA plan.
Work LocationThis is a hybrid position based at TOGETHER!'s headquarters and various Host Homes program locations throughout Thurston County.
Work locations may include host family residences, schools within the North Thurston, Tumwater, and Olympia School Districts, community partner locations, and an approved remote work location. Regular travel throughout Thurston County is required.
Reliable transportation is required for travel between assigned work locations and for transporting youth participants in accordance with organizational policies.
Key Responsibilities Direct Service & Case Management- Provide specialized services to youth and young adults in education, employment and career development, housing, and other designated areas, and support them in achieving their goals.
- Manage a caseload of approximately 8–10 youth throughout the lifecycle of the program. Use TOGETHER!'s intake, follow-up, and other data tracking systems to track youth progress and maintain organized records for reporting requirements.
