Job Summary
Provide professional clinical services under the direction of the Community Based Outpatient Clinic (CBOC) Primary Care Provider. Serve as an Advanced Practice Registered Nurse, delivering urgent and emergent evaluation of episodic and chronic problems; and comprehensive, integrated primary health care that addresses the majority of client health needs on an ongoing basis. Provide assessment, treatment, and evaluation of a comprehensive range of health care problems;
prescribe medications, order diagnostic studies, educate patients and families, and coordinate care.
- Provides primary and preventive medical care within CPT codes and scope of training.
- Delivers care within the scope of practice of national qualifications for Nurse Practitioners.
- Functions within state Nurse Practice Act, Joint Commission standards, VA guidelines, and Inspector General directives.
- Works closely with the Patient Aligned Care Team (PACT) and support staff including daily huddles.
- Emphasizes wellness, prevention, and early detection.
- Supervises support staff in delivery of patient care.
- Maintains universal precautions and infection control practices.
- Provides smooth and timely flow of patients according to VA access standards and triage protocol.
- Performs CPR and assists during respiratory and cardiac arrest procedures.
- Investigates health problems by gathering and assessing historical information.
- Performs physical examinations appropriate to the patient’s history.
- Diagnoses health problems based on history, exam findings, and diagnostic tests.
- Develops a treatment plan tailored to the patient’s needs, including health education and pharmacologic/non‑pharmacologic treatment.
- Prescribes medications as appropriate.
- Evaluates treatment outcomes and modifies plan as needed to achieve goals.
- Makes referrals for specialty evaluation or treatment.
- Directs and coordinates care for walk‑in patients.
- Acts as a model for clinical practice within the clinic setting.
- Sets up and operates therapeutic and emergency equipment (monitor, ECG, oxygen, AED, suction).
- Obtains patient history on new and follow‑up patients and records findings per VA standards.
- Performs initial and periodic physical examinations and records findings.
- Orders diagnostic tests based on chief complaint, physical and emotional findings, and symptoms.
- Demonstrates knowledge of pharmacology and prescriptive authority per state regulations.
- Prescribes and administers medication within privileges and regulations.
- Recommends and follows up on laboratory tests and diagnostic procedures.
- Documents all pertinent patient information in the medical record and VISTA system.
- Keeps the VISTA clinic reminder tracking system current.
- Closes patient charts within 24–72 hours of encounter and places RTC orders promptly.
- Assists in scheduling patients for initial and follow‑up appointments per VA access standards.
- Keeps patients informed of their plan of care, including lab and diagnostic results.
- Provides health educational materials and resources to patients and families.
- Assists the Registered Nurse to ensure complete and accurate electronic documentation.
- Complies with all federal, state, local, Joint Commission, OSHA, VA, and subcontractor safety and operational regulations.
- Participates in the ongoing Performance Improvement Program between VAMC and STGi.
- Ensures all required reports and clinical information are completed accurately and on time.
- Participates in staff development programs.
- Maintains confidentiality of all information and supports patient privacy, rights, and safety.
- Aids in the management/coordination of the CBOC’s day‑to‑day activity.
- Performs other clinical duties assigned within privileges.
- Three years of clinical nursing experience (minimum one year as an APRN). Three years preferred.
- Experience in outpatient care in a Family Medicine or Internal Medicine environment preferred.
- Current and unrestricted DEA License; must obtain and maintain a scope of practice/collaborative agreement with a licensed physician.
- Current and unrestricted Mississippi nursing license.
