- Assess service needs and link the child and family with wrap-around services based on individualized needs (social, educational, vocational, recreational, and related services).
- Serve as an adult role model and work with the family to create a supportive environment through referral, education, and skill development.
- Collaborate with the CPS caseworker to ensure effective service implementation and adjust to changing needs.
- Provide comprehensive case management, including assessment, development of a service plan, referral/linkage to services, and progress monitoring.
- Coordinate with local MCPS staff to initiate contact with the family within 24 hours of referral acceptance.
- Assist eligible individuals in gaining access to medical, social, educational, vocational, and other services.
- Ensure case management services are documented in accordance with programmatic requirements.
- Monitor participant progress and document outcomes; complete initial goals and interventions for the case plan in coordination with the in-Circle Program therapist, MCPS, and the family.
- Facilitate the initial family team meeting with MDHS and conduct subsequent meetings as outlined by the Individualized Service Plan.
- Review and update the Crisis Management Plan (CMP) with each family at team meetings, service plan reviews, or following crises.
- Complete weekly ISP reviews and document progress toward service goals.
- Assess strengths and needs of referred families and utilize community resources to meet their needs.
- Act as a positive role model in all interactions with participants.
- Provide direct case intervention services to children, adolescents, and families in home, school, office, and community settings.
- Coordinate family-driven and community-based social services; provide safe transportation for families as needed.
- Administer satisfaction surveys as assigned and prepare program reports as required.
- Bachelor’s degree in Social Work or a related field; at least two (2) years of experience working with children and families.
- Competence in working with children and families in the child welfare system.
- Competence in completing documentation and reports related to services provided.
- Must be at least 21 years of age, have a valid driver’s license, a good driving record, proof of auto liability insurance, and willingness to use a personal vehicle for organization business.
