Canopy Children’s Solutions in Tupelo, MS, is seeking an In-Circle Program Case Manager to assess needs and connect children and families with wrap-around services, creating individualized service plans and coordinating care.
You will collaborate with CPS and local staff to ensure timely service implementation, provide direct case management in home, school, or community settings, and monitor progress toward goals in coordination with the family and program therapists.
