Visit LPN​/LVN- Per Diem; CovenantCare

Covenant Care at Home is seeking a Visit LPN/LVN. This position is responsible for providing direct patient care under the supervision of a registered nurse.

• Provide direct patient care as defined in the state Nurse Practice Act.
• Implement plan of care initiated by the registered nurse.
• Provide accurate and timely documentation consistent with the plan of care.
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care.
• Participate in coordination of home health services, appropriately reporting the identified needs for other disciplines (HHA, OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor.
• Use equipment and supplies effectively and efficiently.
• Complete and submit all documentation according to agency policy and procedures.
• Participate in personal and professional growth and development.

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years nursing experience.
• Community/home health or medical/surgical experience is preferred.
• Current licensed as an LPN or LVN in the state.
• Ability to comply with accepted professional standards and practice.
• Demonstrates good verbal and written communication, and organization skills.
• Demonstrates good basic computer skills.

#LPN Compensation Pay Range: $33.96 - $47.27 per hour
