Job Overview

We are inspired to serve. Covenant Care at Home is seeking a Visit LPN/LVN to provide direct patient care under the supervision of a registered nurse. The role involves following the plan of care, providing treatments, and working collaboratively with the team to achieve positive patient care outcomes.

• Provide direct patient care as defined in the state Nurse Practice Act.
• Implement the plan of care initiated by the registered nurse.
• Provide accurate and timely documentation consistent with the plan of care.
• Assess and provide patient and family/caregiver education and information pertinent to diagnosis and plan of care.
• Participate in coordination of home health services, reporting identified needs for other disciplines (HHA, OT, PT, MSW, ST, dietician) to the registered nurse and/or clinical supervisor.
• Use equipment and supplies effectively and efficiently.
• Complete and submit all documentation according to agency policy and procedures.
• Participate in personal and professional growth and development.

• Graduate of an accredited practical nurse or vocational nursing program.
• Three years of nursing experience.
• Community/home health or medical/surgical experience preferred.
• Current license as a Licensed Practical Nurse or Licensed Vocational Nurse in the state.
• Ability to comply with accepted professional standards and practice.
• Good verbal, written, and organization skills.
• Basic computer skills.

Compensation Pay Range: $33.96 - $47.27 per hour.
