Job Summary:
Provides coordinated and compassionate home hospice nursing care based on physician orders, applying the nursing process to meet patient needs. Responsible for supervising and evaluating Home Health Aides and LPNs/LVNs while maintaining compliance with agency policies.
Responsibilities:
- Coordinate and deliver comprehensive hospice nursing services to patients in their homes.
- Supervise and assess the performance of Home Health Aides and Licensed Practical/Vocational Nurses.
- Document patient assessments, care plans, and progress within required EMR systems.
- Travel to patient homes within a designated territory to provide care and support.
- Ensure all documentation, including Oasis if applicable, is completed accurately within 24 hours.
- Participate in periodic office meetings and team collaborations to support patient care.
- Hold a valid RN license in the state of assignment, in hand.
- Possess a minimum of 1 year of recent adult home health or hospice experience.
- Demonstrate proficiency with Home Care Home Base (HCHB) and Oasis (for RNs, PTs, OTs).
- Have reliable transportation, a current driver's license, and appropriate liability insurance.
- Be willing to travel up to 75 miles from the home clinic as needed.
- Be able to perform home visits and participate in on-call rotations.
