LVN ​/ LPN – LTC​/SNF

LVN/LPN Position

Seeking a dedicated LVN/LPN to provide quality nursing care in a long-term care or skilled nursing facility setting. This role involves collaborating with a healthcare team to deliver patient-centered services and ensuring compliance with healthcare standards.

Responsibilities:

• Administer prescribed medications and treatments to patients according to established protocols.
• Monitor and record patients’ health status, including vital signs and responses to treatments.
• Assist with patient assessments and report any changes in condition to the supervising nurse.
• Follow infection control procedures and maintain a clean, safe environment.
• Communicate effectively with patients, families, and healthcare team members to coordinate care plans.

Qualifications:

• Current valid LVN/LPN license with the state licensing board.
• Minimum of 1 year of experience in a long-term care or SNF environment.
• Demonstrate strong clinical and assessment skills.
• Ability to follow detailed instructions and maintain patient confidentiality.
• Basic life support (BLS) certification required.
