Job Description for Transition Specialist

The Transition Specialist supports special education students, families, and staff in planning and implementing effective transition services. This supplemental role is designed to provide targeted assistance in preparing students with disabilities for post-secondary education, employment, and independent living. The Transition Specialist collaborates with school personnel, community agencies, and families to ensure compliance with IDEA transition requirements and to support student success beyond high school.

Full-Time Salary Range: MCBOE Supplement Schedule

Qualifications:

  • Bachelor's degree from a regionally accredited college or university
  • Professional teaching certificate issued by the Alabama State Department of Education prior to starting date
  • At least three years of successful teaching experience in special education or related areas
  • Fingerprint clearance with the Alabama State Department of Education prior to the start date

Essential Duties and Responsibilities:

  • Collaborate with IEP teams to develop, review, and monitor transition goals, objectives, and services that align with each student's postsecondary interests, strengths, and needs.
  • Conduct and interpret age-appropriate transition assessments to identify student preferences, interests, and skills for future planning.
  • Assist teachers in embedding transition and career readiness skills into classroom instruction.
  • Provide guidance to students on employment skills, self-advocacy, self-determination, and independent living.
  • Establish and maintain partnerships with agencies, vocational rehabilitation, workforce development, and post-secondary institutions.
  • Coordinate and support work-based learning opportunities, such as job shadowing, internships, and community-based instruction.
  • Assist families in understanding and accessing transition-related resources, college readiness, financial aid, and adult services.
  • Organize and facilitate student and family workshops, informational sessions, and transition fairs.
  • Train and support staff in best practices for transition planning and compliance with IDEA transition requirements.
  • Collect, maintain, and report accurate data and documentation related to student transition goals, activities, and services.
  • Serve as a liaison between the school district and outside agencies to ensure continuity of services during and after high school.
  • Support the district in developing and refining transition-related procedures, forms, and resources to improve program effectiveness.
  • Perform other duties as assigned by the Director of Special Education or other administrator.

  • Ability to establish and maintain cooperative and effective working relationships with others. Advanced understanding and knowledge of current technology and computer software.

Physical Requirements:

While performing the duties of this job, the employee may be required to sit for extended periods of time. The employee must also talk, hear, stand, and walk. The employee may be required to use their hands to finger, handle, feel, and/or reach. The employee is occasionally required to stoop, kneel, or crouch. The employee must occasionally lift and/or move up to 25 pounds.
