Overview

Central Alabama Veterans Health Care System is actively hiring for one (1) Social Worker for the Caregiver Support Program, doing the role of a Case Manager. Located at the Tuskegee, AL facility, the primary responsibilities are to provide clinical evidence based services and interventions; assist with program development; caregiver, Veteran and staff education on caregiver issues; community outreach; resource development; continuous quality improvement activities;

and evaluation/consultation.

• This social worker is assigned to the Caregiver Support Program at the Central Alabama Veterans Health Care Center (CAVHCS). The social worker is responsible for providing social work services to the Veteran residents of this area. The incumbent is a member of an interdisciplinary team, and is primarily responsible for completing psychosocial assessments, providing caregiver, Veteran and staff education on caregiver issues;
  
  engage in community outreach and resource development; continuous quality-improvement activities, and evaluation/consultation.
