Peer Intake Navigator

Position Summary

The Peer Intake Navigator is responsible for providing intake services to newly referred AES Guests. The Peer Intake Navigator will facilitate a warm welcome to AES, verify referral information from the referring agency, provide an AES site tour to guest, conduct an intake and assessment with new guest, review site guidelines and expectations for living at AES, and provide an on‑site services overview with the guest.

The Peer Intake Navigator will ensure data accuracy and enter intake information and documentation into the internal guest database and HMIS (Homeless Management Information System). The Peer Intake Navigator will be supervised and work closely with the Program Supervisor and Program Lead to ensure the move‑in of new guests is a seamless transition from homelessness to emergency shelter at AES.

Essential

Functions
  • First point of contact for new guest referrals or inquiries about referrals.
  • Conduct phone screening with referring agencies.
  • Cross‑reference indefinite exit list.
  • Schedule intake date and time.
  • Review referral information.
  • First point of contact upon arrival on‑site.
  • Place new guest's items in bug zapper.
  • Provide a warm welcome and site tour.
  • Create a safe and inviting space for guest to engage in the intake process: review program guidelines & expectations, obtain Release of Information (ROI) for agencies guest is linked to, conduct homelessness assessment, go over Occupancy Agreement, review AES information for living in communal space: shower, bathrooms, laundry, meals, shuttle times, mail, bikes, pets, vehicles, couples dorms, programming services.
  • Assist guest with acquiring guest  move‑in kits: hygiene and linen.
  • Show guest bed assignment.
  • Conduct New Resident Orientations.
  • Maintain intake tracking system to ensure it is current.
Miscellaneous Duties
  • Participate in AES “all” staff meetings.
  • Participate in Programs team meetings, case conferencing, and operations huddles as needed.
  • Participate in professional development trainings.
  • Participate in Homeless Service staff meetings and trainings.
  • Participate in Social Service and Community meetings such as CES weekly match meetings.
  • Maintain a positive working relationship with all TSA staff and external community partners.
  • Perform other duties as directed.

PAY RATE: $20‑$22.00/hr.

Working Conditions

Ability to walk, stand, bend, squat, climb, kneel and twist on an intermittent or sometimes continuous basis. Ability to grasp, push, pull objects such as files, file cabinet drawers, and reach overhead. Ability to operate computer, fax, and telephone. Ability to lift up to 40 lbs.

Minimum Qualifications
  • AA, BA, or equivalent degree (preferred).
  • 2 years of HMIS experience (preferred).
  • Excellent interpersonal and professional communication skills to deal with residents, co‑workers/volunteers, management, and community members.
