Position Summary
The Case Manager I provides holistic, strength‑based, harm reduction case‑management services specializing in housing. Responsibilities include removing barriers to housing readiness, assisting guests with appointments and documentation, developing housing plans, budgeting, linking guests to resources, applying for housing, locating housing, and advocating for housing. The role involves visiting housing options, building rapport with landlords and property managers, and coordinating guest move‑out into permanent housing.
The Case Manager also participates in weekly Coordinated Entry System (CES) meetings to update on guest status.
Pay Rate
$25.00/hr – $27.00/hr.
Essential Functions
Case Management
- Ensure timely, efficient, and effective holistic case management.
- Maintain an active case load.
- Work with guests to remove barriers to housing focus.
- Create and perform needs assessments and case plans.
- Meet with each guest on the case load four times a month.
- Maintain case load tracking system current.
- Enter guest case notes weekly in internal and external databases.
- Assist guests with obtaining required housing documents.
- Facilitate weekly life‑skills groups on budgeting, tenant responsibilities, independent living, and housing program information.
- Use conflict management, de‑escalation, and crisis intervention techniques in emergencies.
Housing Navigation
- Develop individualized guest‑centered housing plans.
- Conduct bungalow/bed space visits to maintain guest engagement.
- Research and visit housing options for low‑income individuals and seniors; build rapport with landlords and property managers.
- Maintain a list of current vacancies: low‑income apartments, VASH, Section 8, room for rent, room and board, and sober living homes.
- Assist guests in locating and securing permanent housing via outreach.
- Meet with each guest on the case load four times a month.
- Create exit plans and coordinate move‑out into housing.
- Assist in training new hires, interns, and volunteers; allow them to shadow.
Record Keeping
- Maintain documentation including housing documents, guest goals, monthly reports, and physical files.
- Enter case notes into HMIS, CES, Well Sky, and Cal Optima timely.
- Exit guests from case load and databases; close files.
- Process Cal Optima (CalAIM) billing submissions by deadlines.
- Communicate guest status, challenges, and needs to supervisors and agencies.
- Participate in programming, case management, guest meetings, and staff trainings.
- Maintain positive relationships with TSA staff and community partners.
- Perform other duties as assigned by Supervisor.

Working Conditions
Ability to walk, stand, bend, squat, climb, kneel, and twist intermittently. Ability to grasp, push, and pull objects such as files and file cabinet drawers. Ability to operate computer, fax, and telephone. Ability to lift up to 40 lbs.
Minimum Qualifications
- Demonstrated understanding of The Salvation Army’s Christian mission due to direct contact with program participants.
- BSW or equivalent degree (preferred).
- Minimum one-year experience in case management with recovery, mentally ill, and/or homeless populations.
- One‑year experience with HMIS (preferred).
- Experience with tenant/landlord relationships.
- Good physical and mental health to meet job demands.
