Duties
* Facilitate continuity of care through a multidisciplinary approach in behavioral treatment using a population-based care management model in support of primary care providers.
* Educate patients on application of treatment plan, to include medication, risk factors, or interactions with other substances.
* Perform independent, comprehensive, telephonic, and in-person clinical triage of Behavioral Health symptomology.
* Document patient information using an electronic health medical record.
* Monitor patient progress and symptom severity by conducting a structured telephonic interview and using validated tools (e.g. PHQ-9, GAD-7 and PCL-5). Requirements Conditions of employment
* Appointment may be subject to a suitability or fitness determination, as determined by a completed background investigation.
* This position requires the completion of a pre-employment Physical Examination and an annual examination thereafter to ensure the continued, required level of physical health and ability or fitness to perform the duties of the position.
* This position is subject to a pre-employment screening, and random testing thereafter, to include testing based on reasonable suspicion and testing due to direct involvement with an on-duty accident.
* In the event of severe weather conditions or other such emergency type situations (natural or man-made disaster) the incumbent is required to report to work or remain at work as scheduled to support mission operations.
* Appointment to this position is subject to a one year probationary period unless the appointee has previously met the requirements as described in 5 CFR Part 11.
* This position has a Temporary Duty (TDY) or business travel requirement of 15% of the time.
* This position requires Basic Life Support training certification from, or in accordance with, the American Heart Association and American Red Cross.
* This position is required to be screened for and obtain necessary immunizations/vaccinations (hepatitis, influenza, etc.), unless exempted by a licensed physician.
* This position has a mandatory seasonal influenza vaccination requirement and is subject to annual vaccination.
* This position requires a current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States.
* This position is designated Tier 3, Non-Critical Sensitive, Moderate Risk and requires the incumbent to obtain and maintain a Secret Security Clearance Level as a condition of employment.
* This is a drug testing position. Qualifications Who May Apply: US Citizens In order to qualify, you must meet the education and/or experience requirements described below. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, Ameri Corps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; student; social). You will receive credit for all qualifying experience, including volunteer experience.

Degree: A graduate or higher level degree, bachelor's degree, associate degree, or diploma from an accredited professional nursing educational program is required. This education must have been accredited by the Commission on Collegiate Nursing Education, Council on Accreditation of Nurse Anesthesia Educational Programs, Accreditation Commission for Midwifery Education, or an accrediting body recognized by the U.S. Department of Education at the time the degree was obtained.

Licensure: A current, active, full, and unrestricted license or registration as a professional nurse from a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States; and you must have passed the National Council Licensure Examination. Foreign Degree: A degree from a Foreign Nursing School. Official certification from the Commission on Graduates of Foreign Nursing Schools is required for individuals who graduated from foreign nursing schools.
Exemption:
You are exempt from the basic requirements above if you are a current federal employee occupying a position in the 0610 occupational series and have been continuously employed in this occupational series since September 27, 2017 or before.
