Job Summary
As one of the Top 8 Most Innovative Healthcare Systems in Becker’s Hospital Review, we’re working to create a national model for improving health. You will serve as a Nurse Home Visitor, acting as a case manager and implementing the standard protocols of the Nurse Family Partnership.
Location & ScheduleWork from home (Pennsylvania). Days shift, 20 scheduled weekly hours (8:00am to 4:30pm) with occasional evenings for client needs.
Responsibilities- Act as a case manager and implement standard protocols for families enrolled in the Nurse Family Partnership.
- Assess the physical, emotional, social, and environmental needs of women and their families.
- Initiate and coordinate visits based on standardized protocols, including setting goals and outcomes.
- Provide anticipatory guidance and instruction to mothers and significant others regarding pregnancy, infants, and toddlers.
- Use therapeutic relationships, reflection, and motivational interviewing techniques.
- Complete and submit all documentation within 48 hours.
- Provide discharge planning when appropriate.
- Educate referral sources and the public about the Nurse Family Partnership.
- Develop and maintain community relationships to support client referrals.
- Participate in review and analysis of reports to assess achievements and areas for improvement.
- Implement Telehealth services according to national office guidelines and agency policy.
- Work in a home visiting environment, serving the Northumberland County area.
- Bachelor's Degree in Nursing (preferred).
- Minimum of 2 years of clinical experience (preferred).
- At least 2 years of RN OB and/or Pediatric experience (strongly preferred).
- Licensed Registered Nurse (Pennsylvania) with a valid RN license.
- Basic Life Support Certification.
- Previous experience in home visits, behavioral health, or public health is helpful.
- Strong communication and interpersonal skills.
