Clinical Liaison Horizon Home Health & Hospice is seeking a full‑time Clinical Liaison to represent our home‑health and hospice services throughout the Twin Falls region. This role is ideal for a clinician with an active COTA, LPN, PTA, or RN license who excels at building relationships, educating referral partners, and driving referral growth. The Clinical Liaison serves as a key connector between Horizon and the community—engaging hospitals, SNFs, clinics, and physician groups to ensure patients receive timely access to home‑based care.

Key Responsibilities- Build and maintain strong relationships with referral sources including hospitals, SNFs, clinics, and physician groups.
- Educate partners on Horizon’s home‑health and hospice services, clinical capabilities, and patient‑centered care model.
- Identify referral opportunities and collaborate with partners to ensure timely, appropriate patient transitions.
- Leverage clinical expertise (COTA, LPN, PTA, or RN) to communicate patient needs and service benefits effectively.
- Conduct on‑site visits, presentations, and outreach to strengthen Horizon’s presence in the community.
- Work closely with intake, clinical leadership, and field teams to ensure seamless onboarding of new patients.
- Track referral trends, outreach activities, and relationship‑building efforts to support strategic growth.
- Represent Horizon with professionalism, empathy, and CAPLICO‑aligned leadership.
- Active COTA, LPN, PTA, or RN license in Idaho (required).
- Strong communication, relationship‑building, and presentation skills.
- Experience in home health, hospice, or post‑acute care preferred.
- Ability to work independently and manage a dynamic outreach schedule.
- Reliable transportation and valid driver’s license.
Full‑Time Schedule Monday-Friday Normal business hours Community‑based travel throughout the Twin Falls region
Compensation & IncentivesCompensation Details
Base salary: $70,000-$90,000
