Overview

The Idaho Health & Welfare’s Continuum Care Bureau is hiring a Family Support Clinician (FSC) for our Twin Falls field office. FSCs are an essential component of the Continuum Care Bureau's mission: ensuring that youth receive the right level of care, the appropriate treatment, and the necessary duration of support. FSCs provide clinical support and psychoeducation to birth, foster, post‑adoption, and other caregivers, aiming to stabilize youth within their communities, prevent placement disruptions, and assist with healthy Continuum Care transitions during discharge.

Some travel may be requested.

Responsibilities
  • Provide clinical support and psychoeducation to birth, foster, post‑adoption, and other identified caregivers by offering targeted sessions that address immediate concerns and equip caregivers with tools to manage youth behavior effectively.
  • Assist in maintaining stability within community placements by identifying early signs of distress and intervening with strategies to mitigate potential disruptions (in person, via telecommunication, or phone support).
  • Utilize knowledge of community resources to suggest services in home, office, or virtual settings to maximize accessibility and accommodate the diverse needs of families.
  • Deliver clinical support and psychoeducation to caregivers during youth transitions from continuum care, ensuring they understand behavioral expectations and best practices for a smooth reintegration.
  • Focus on stabilizing youth post‑discharge to prevent re‑entry into continuum care through structured check‑ins and reinforcement of coping mechanisms.
  • Coordinate with the discharging treatment team to ensure a “soft” handoff and continuity of care by facilitating a collaborative transition plan that aligns with the youth’s therapeutic goals.
  • Assess youth treatment readiness and overall needs through structured assessments and goal‑setting discussions, depending on age.
  • Provide short‑term clinical intervention to support treatment planning by reinforcing therapeutic strategies and addressing immediate emotional or behavioral concerns.
  • Maintain awareness of service availability in the community, ensuring services are accessible in multiple formats (in‑home, in‑office, virtually).
  • Ensure families have access to 24‑hour helplines for crisis support, offering immediate guidance and intervention when necessary.
  • Have FSC services reviewed by a supervisor every three weeks to assess progress and determine next steps, ensuring accountability and continuous improvement.
  • Document all interactions, support provided, and coordination efforts to maintain continuity and effectiveness of care, using standardized tracking systems and case notes.
  • Provide timely updates to the regional Case Manager through structured reports and scheduled check‑ins.
  • Engage in regular communication that includes the initial engagement of services, progress updates, and confirmation of transition to long‑term community‑based support when applicable.
Qualifications
  • Master’s degree (or higher) in a direct clinical practice Human Services field such as social work, psychology, marriage and family counseling or closely related field in accordance with Idaho Codes: 54‑3206, 54‑3402, 54‑3405, and 54‑3214.
  • Requires licensure in a professional specialty:
    Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in accordance with Idaho Code 54‑3206;
    Licensed Professional Counselor (LPC) in accordance with Idaho Code 54‑3405;
    Marriage and Family Therapist (LMFT) in accordance with Idaho Code 54‑3405.
  • Will not receive relocation reimbursement and the department does not sponsor work visas.
