CHRISTUS Health is hiring an Ameri Corps Community Connector I in Tyler, Texas. This full-time role involves working with individuals to provide culturally competent screening and referral services, developing care plans, and educating them on health and community resources. Candidates should have strong organizational and communication skills, a high school diploma, and be comfortable working in community settings. Bilingual candidates are preferred, and a Community Health Worker Certification must be obtained within 6 months of hire.
