Physical Therapist-Home Health
Pride Health is hiring a Physical Therapist-Home Health to support our clients in the medical facility. This is a temporary assignment opportunity and a great way to start working with a top-tier healthcare organization!
Job Type: Contract
Location:
Tyler, TX
Shift: Monday–Friday 8:00 am to 4:30 pm
Hours:
40 hrs/week
Coverage Areas:
Hawkins, Mineola, Lindale
Productivity Expectation: 30 points per week
Work Setting:
On-site (clinic-based)
Mileage Reimbursement:
Yes (between patient visits, from first to last visit)
Pay: $50 to $56/hr on W2
* Pay varies based on experience
Key Responsibilities:
- Perform comprehensive patient assessments in home health and hospice settings
- Develop and implement individualized Plans of Care (POC)
- Complete required documentation, including:
Start of Care (SOC), Resumption of Care (ROC), Recertifications and reassessments - Provide therapeutic interventions to improve patient function and independence
- Supervise and collaborate with Physical Therapist Assistants (PTAs)
- Educate patients and caregivers on treatment plans and home exercise programs
- Ensure compliance with all clinical and regulatory standards
Required Qualifications:
- Active Physical Therapist (PT) license
- Prior experience in Home Health (HH)
- Experience with Homecare Homebase (HCHB) documentation system
- Strong clinical assessment and care planning skills
