Job Shift: Day The Center at Grande is looking for a Case Manager to join our team!

The Center at Grande is a skilled nursing facility that serves patients needing physical rehabilitation and/or complex nursing care. Our highly qualified staff provides care in a beautiful hotel like atmosphere to help our patients achieve strength and functionality. Skilled-nursing care, family involvement, psychological care and discharge planning complement a well-rounded, individualized program and encourages each patient’s transition to their highest functional level.

• Track and record patient case information, progress, and evaluate recovery.
• Meet with patient and their families to understand their needs and recommend appropriate care with treatment plans, while providing support and reassurance.
• Work with team members to ensure discharge planning goals and objectives are developed.
• Schedule appointments, and coordinate care

• Excellent verbal and written communication with professional phone etiquette.
• Minimum of 1 year Case Management experience preferred
• Customer service oriented
• High attention to detail
• High School diploma or GED
• Must be able to pass a pre-employment background check and drug screen
