Job Details / Responsibilities:
- Provide comprehensive clinical pharmacy services with emphasis on ICU and Internal Medicine patients, including pharmacokinetic dosing, therapeutic monitoring, and antimicrobial stewardship
- Review, interpret, and verify medication orders for appropriateness, safety, and efficacy while collaborating with physicians and interdisciplinary teams
- Compound and dispense medications, including sterile products, chemotherapy, and parenteral nutrition as needed
- Monitor drug therapy regimens for interactions, contraindications, adverse reactions, and renal/hepatic adjustments
- Participate in daily rounds, code responses, and high-acuity patient care in critical care and medical units
- Provide drug information, patient/family education, and discharge medication reconciliation
- Supervise pharmacy support personnel and ensure compliance with all regulatory standards
- Rotate every 3rd weekend and cover occasional night shifts for staffing needs while maintaining efficient, timely services
- Doctor of Pharmacy (PharmD) or BS Pharmacy degree required
- Active Pharmacist license (or eligible) in Texas, with licensure within 60 days of start
- 3–5 years hospital experience or completion of PGY1 (or PGY2) residency required
- Must have ICU and/or Internal Medicine clinical experience
- Current BLS (ACLS preferred) or obtained within 180 days
- Strong critical thinking, communication, and problem-solving skills in a fast-paced environment
- Ability to work full-time day shifts (primarily 8:00 AM–4:30 PM M-F) with every 3rd weekend and occasional nights
