Choice Hospice is seeking an experienced Hospice RN Evaluator/Admission Nurse to provide compassionate, patient-centered care to individuals with life-limiting illnesses. In this role, you will complete hospice evaluations and admissions, determine eligibility, develop individualized plans of care, and educate patients and families while ensuring a smooth transition into hospice services.

What You'll Do- Complete hospice evaluations, admissions, and comprehensive nursing assessments.
- Determine hospice eligibility using admission and recertification criteria.
- Develop, implement, and update individualized plans of care.
- Coordinate care with physicians and the interdisciplinary hospice team.
- Educate patients and families on disease progression, hospice philosophy, medications, symptom management, nutrition, and end-of-life care.
- Assess physical, emotional, psychosocial, spiritual, and environmental needs.
- Evaluate caregiver readiness and provide ongoing support and education.
- Supervise and provide clinical direction to Hospice Aides and LPN/LVNs.
- Document assessments, admissions, care plans, and clinical progress accurately and timely.
- Communicate changes in patient condition to the physician and interdisciplinary team.
- Participate in case conferences and quality improvement initiatives.
- Perform on-call responsibilities as assigned.
- Current Registered Nurse (RN) license.
- Current CPR certification.
- Valid driver's license and reliable transportation with current auto insurance.
- Minimum of 2 years of hospice nursing experience preferred.
- Home Care Home Base experience preferred.
- Excellent assessment, communication, and critical thinking skills.
- Ability to work independently while collaborating effectively with an interdisciplinary team.
